Students should return their completed lunch forms and FULL two-week payment in a SEALED envelope with their name and teacher clearly marked on the outside. No funds will be accepted outside of this two-week cycle system.

If your child is not having lunch on a particular day please circle NL for "No Lunch".

All lunch options come with your choice of milk. Please select the type of milk
(1%, skim, or fat free chocolate) that you would like your child to have.

If your child is NOT having lunch but you would like to purchase a milk for them please circle MO for "Milk Only" and select the type of milk you would like to purchase.

If you would like to purchase an extra slice of Rocky's Pizza for your child on Friday's for an extra $1.90 please circle option 1 AND ES for "Extra Slice".

All inquiries should be directed to your child's Homeroom Teacher.
