The below process applies only to NHS GRADUATES who are requesting a transcript.  If you are a CURRENT STUDENT, please see your School Counselor. 

All requests for graduate transcripts, test scores, or educational verification requests will be fulfilled by NHS School Counseling Secretary, Mariel Pasquale: 

Step 1: 

Please complete the form below for graduate transcripts, health records, and/or test scores:

Complete your educational verification request form (*We do not provide disciplinary records) 

Step 2: 

Once the form is completed, please email, fax, or drop off the form to Mariel Pasquale in the NHS School Counseling office
